Hearing and vision screenings will start in September for students in PreK, Kindergarten, 1st, 3rd, 5th, 7th, and 9th grades. If you prefer that your child not participate in these screenings, please complete the opt-out form below and either email it to the CPSB School Nurse at jsharp@cpsbla.us or deliver it to your child's principal.
Additionally, if your child is not in one of the specified grade levels but you would like them to be screened, you may request a screening by contacting the CPSB School Nurse at the email address above.
